McVay Tissue Repair

  • Approximate the edge of the transversus abdominis aponeurosis to the Cooper ligament with interrupted Prolene sutures.
  • At the medial aspect of the femoral canal, a transition suture is placed to include the Cooper ligament and the iliopubic tract.
  • Working laterally, the transversus abdominis is secured to the iliopubic tract.
  • A relaxing incision is made by reflecting the external oblique aponeurosis superiorly and medially to expose the anterior rectus sheath. The incision is made in a curvilinear direction, 1 cm above the pubic tubercle toward the lateral border of the anterior sheath.
  • This is a tissue-based repair that provides appropriate coverage of a femoral hernia defect.
