Adrenalectomy
- Always test for function prior to removal (metanepherines, BMP, etc).
- Indications
- Non-homogenous/complex, >4-6cm, HU >10, <50% washout at 10-15min, functional, enlarges >1cm while following, +FNA (don’t bx).
- Right adrenalectomy → requires Kocher + mobilize right lobe of liver.
- Left adrenalectomy → mobilize spleen & tail of pancreas (risk of injury). Phrenic vein passes medial to gland & into left renal vein near/with left adrenal vein.
