Zenker's Diverticulectomy
- Beach chair position with shoulder roll & head turned to the right.
- Left neck incision anterior to SCM.
- Retract strap muscles laterally & trachea medially.
- Dissect down to esophagus and proximal trachea taking care to preserve RLN in tracheoesophageal groove.
- Place a 56F bougie.
- Resect diverticula with a TA stapler.
- PERFORM CRICOTHYROID MYOTOMY!
- Close in layers.
- Alternative → transoral stapler if >3cm (can invert & staple OR staple straight down to create a common channel).
